Sfront is set up to link into MIDI files or local or network MIDI
streams, you can write SAOL that reacts in arbitrary ways to a
local keyboard or a MIDI file. See:
http://www.cs.berkeley.edu/~lazzaro/sa/index.html
for the whole story, or these links touch on different parts of
MIDI + Structured Audio:

http://www.cs.berkeley.edu/~lazzaro/sa/sfman/user/cmdline/index.html#cin
http://www.cs.berkeley.edu/~lazzaro/sa/sfman/user/rtime/index.html#rt
Several of the examples that ship with sfront use MIDI files or
take in input from a local MIDI Jack or a network MIDI input, see:
http://www.cs.berkeley.edu/~lazzaro/sa/sfman/user/network/
for the network musical performance story.
