The asterisk opensource pbx should do what you want with maybe a bit
of hacking. See http://www.asteriskpbx.org

The x100p card that digium sells is actually just a winmodem,
which you can find on ebay much cheaper. Connecting asterisk to your
streaming server should be possible, but if it is a problem asterisk
can output to a soundcard.
