There are few possibilities to look at:

1) Make sure the same sound card is used both for input and output. Two
(separate) sound cards never run precisely at the same rate unless they
are syncronized together in some special way. If there is any drift in the
rates this will cause underruns or overruns. Maybe once per few seconds or
minutes.
2) If the system load is too high (or the buffer size set to too
small) then there will be buffer underruns. Each of them will cause a
pause/click.
3) There is some bug in the application logic and it just let's the buffer
to drain before writing new data.
