mine reads as

options snd-usb-audio index=2,3,4,5,6
vid=0x0582,0x0763,0x0763,0x0582,0x0ccd pid=0x0074,0x1033,0x0117,0x000

and works fine.

Note I just found an issue concerning modules-update. Read
your /etc/modules.conf carefully. The modules-update script will
include *any* file found in /etc/modules.d/, regardless if it reads as
alsa.backup or alsa.orig.tar.gz. Having backup files in this directory
will cause your /etc/modules.conf to be messed :) .
