Whats the exact mathematical background of good pitch shifting ?

you convert (eg 256) samples with fft, then you crunch or enlarge the frequency
scale and convert it back to samples

but

* the minimal frequency converteted depends on the number of samples
* I dont know whether the result would be good if you took too much
samples(65536)

* If you dont do a phase alignment, the result sounds very cracky (not
monotonically)
* If you do a phase alignment between the samples, the result values might go
out of the integer boundary.

so whats a good way to do pitch scaling ?
