One quibble: the ground wire and the screening mesh are one and the
same in every cable I've looked at, both unbalanced and balanced.

So an unbalanced cable has a single conductor, surrounded by insulation,
surrounded by ground conductor (either a braided mesh or a foil wrap).
Balanced is built the same way, except there are two insulated
conductors in the middle.

Exception: I think some "snake" cables have ground shields for each individual
balanced pair, and an additional shield wrapped around the whole thing.
