This looks as if the driver sends data for more channels than the device
expects. How many playback/capture channels does Jack report, and how
many is the device supposed to have at that frequency? Could you please
check this for all frequencies?

It looks as if you don't have Colin's problem of the kernel crashing;
you might want to avoid upgrading to 2.6.32.

The device doesn't send capture data ...

... and doesn't react to any message; probably it got thoroughly
